Factors Associated with the Successful Outcome of Lumbar Nucleoplasty for the Treatment of Internal Disc Derangement

- BACKGROUND: Lumbar nucleoplasty is an effective treatment for internal disc derangement or deruption(IDD) of lumbar disc. We evaluated predictive factors associatied with the successful outcome of lumbar nucleoplasty.
- METHODS: Lumbar nucleoplasty using YES DISC® guided by fluoroscopy was conducted for lumbar IDD by one pain physician. Successful outcome was defined as more than 50% pain relief on the numerical rating scale pain score during the 6-month follow-up period. The relationship between outcomes and independent variables, including patient demographics, comorbid diseases, pain duration, numbers and level of the affected disc, Dallas Discogram Description, presence of other spine disease such as spinal stenosis, were investigated using multivariable analyses.
- RESULTS: Of 80 patients, 47 experienced a successful outcome after YES DISC® procedure. Higher Dallas discogram scale and the absence of coexisting lumbar disc disease were related with the positive outcome. There were no other statistically significant between-group differences in the other factors. No serious complications related to nucleoplasty occurred.
-CONCLUSIONS: In this study, the success rate of lumbar nucleoplasty using YES DISC® was 58.8% in patients with IDD. The higher Dallas Discogram scale, the absence of coexisting lumbar disc disease were positive predictive factors of successful lumbar nucleoplasty.
